Depression

Bipolar Disorder

Returning Patient Appointment- Not Seen Within the Last Year (40-60 min): $400
Follow Up Appointment (20 min): $175
Follow Up Appointment (30 min): $200
Follow-up Appointment with Therapy (40 min): $250
Paperwork/Forms Completion Appointment for 2+ pages (20 min) $175
Paperwork/Forms Completion Outside of an appointment for 2+ pages (i.e., FMLA, Disability, etc.) $400/hr
Paperwork/Forms Completion Appointment for 1 page (20 min) $175
Paperwork/Form Completion Outside of an appointment for 1 page (i.e., for School, Medication Administration, etc.) $25

We accept cash, check, and all major credit cards as forms of payment. We also accept payment directly on my site via this page.
